Please copy and paste and answer the following questions and then submit. Please use as much detail as possible as this gives us a better idea of the type of home our puppy would be going to. One word answers like repeated "Yes/No" does not give us much insight and you are more likely to not be approved:

What size are you interested in mini sized Cockapoo, Standard Sized Cockapoo?
Are you a breeder looking for breeding rights?
Are you applying for a pet position or guardian ship position under our breeding program?
Where do you live?
Have you welcomed a puppy to your home in the last year that has been suspected of having or diagnosed with parvo?
What time frame are you looking at to welcome your new puppy?
Could you please tell me a little about yourself and your family?
Have you ever owned a dog before?
Have you ever rehomed a dog before? if so why?
What is you work schedule?
What is your living situation (home/yard)?
How many hours would your puppy/dog be on it's own?
Who will be the main care giver for the puppy/dog?
Are you able to meet the financial cost of owning a Cockapoo (good food, emergency vet care, regular grooming, etc..)?
Are you willing to enroll and take your puppy to obedience class?
Do you plan on your puppy/dog living indoors or outdoors?

Are you sure you are ready for the lifelong commitment of owing a dog?
Why do you believe that this breed is the right fit for you?
What are you looking for in a dog?
Will your puppy need to be shipped? If so where?
